One of the circles that the u joint goes in is cracked halfway through and warped

Because fast lifted truck.
That's called a yoke.
They can be welded/cleaned up on a steel driveshaft, though you may have to have it rebalanced afterwards.

got a new yolk welded on, driveshaft was bent, they fixed it and balanced it, new u-joints... no shudders and vibes, did a 10-105 run, also got her up to 115, and all is smooth... no shudders or vibes when letting off the gas
